Being the main contractor in relation to a construction site where construction work was being carried out that exceeded a height of 6 metres above ground level, failed to ensure that there was displayed at the main entrance to the site a safety warning sign bearing the words "SAFETY HELMET AREA. SAFETY HELMETS MUST BE WORN AT THIS SITE".
