|Description of Breach(es)||
Failed to make arrangements for ensuring, so far as is practicable, that cleaning of plant at the workplace is carried out in a manner such that employees are not exposed to hazards.
On 29 May 1996 two employees were cleaning a Mitsubishi Langston Printer. The machine was shutdown, although the rollers still rotated slowly even with the machine shutdown. One of the employees was using a rag to clean one of the print cylinders when the rag became caught by the nip points between the roller and print cylinder. The Stereo Lock on the print cylinder came around and severed two of the employee's fingers.
