On 18 October 1996, Mr John Jessett, a roof plumber, worked at the Rydges Hotel site in a precarious position at a height (approximately 4-5 metres at one end to about 9-10 metres at the other end of the roof) without the use of the fall arrest system that was supplied by his employer to prevent and reduce the likelihood of injury in the event of a fall.
