Failing to notify the WorkSafe Western Australia Commissioner of injuries to employees, as specified in Regulation 201(1)(e).

Background Details

[1] A truck driver/labourer employed by the Shire, received a lower back injury on 26 September 1995 when he was lifting a bag of cement onto his truck which resulted in him having more than 10 working days off.

[2] A truck driver/labourer employed by the Shire, received a lower back injury on 30 May 1996, when he was assaulted by a member of the public as he carried out his work duties, requiring him to be off work for more than 10 working days.

[3] A truck driver/labourer employed by the Shire, received a shoulder injury on 3 November 1995 whilst he was trying to start a lawn mower, and resulted in him being absent from work for more than 10 working days.
