On 12 February 1997 Mr Smith operated a skid steer loader while another employee (work colleague) was sitting on the machine's bucket. As the machine started Mr Smith had his foot on the bucket control and caused the bucket to fold back and the other employee to loose his position on the bucket. Mr Smith's colleague suffered a torn scrotum and bruised back. |
